About your business

The basic information about your forfeited entity. Match what's on your home-state formation documents.

Use the exact name on your home-state formation documents, including "LLC" or "Limited Liability Company".
If your entity uses a different name in Maryland (often required when the legal name conflicts with another MD entity), enter it here. Must include "LLC" or "Limited Liability Company".
The main business address of your entity, typically in your home state.
A brief description of what your business does in Maryland. Required by SDAT.
Is your entity registered as a Series LLC in your state of formation?
3

Maryland status

Details specific to your Maryland forfeiture. Find your SDAT ID at Maryland Business Express.

Found on your SDAT entity record. Usually starts with a letter (Z, F, D, etc.) followed by numbers.
Each missed Form 1 / Personal Property Return adds ~$300 in state fees. We bill these at actual state cost with no markup.
Heads up: Maryland charges a $200 penalty when a foreign entity has been operating without registration. This is a state fee paid directly to SDAT (not a fee from us) and we'll include it in your written quote so there are no surprises.
Important: SDAT requires Maryland Comptroller tax clearance before processing reinstatement when state tax is owed. We coordinate this on your behalf as an add-on service. Cases requiring tax clearance may extend beyond the 24-hour timeline; we'll quote a realistic timeline once we know the specifics.
4

Maryland resident agent

SDAT requires a Maryland resident agent with a physical address. P.O. boxes are not accepted.

If you don't currently have one or need to switch, leave this blank. We can recommend an MD resident agent service or appoint ourselves as part of the filing.
5

Home-state Certificate of Good Standing

Maryland requires a current certificate from your home state, dated within 60 days. Your home-state entity must also be active and compliant.

Add-on services available: If you don't have a current Certificate of Good Standing, we can obtain one from your home state on your behalf. If your home-state entity is also forfeited, we can coordinate home-state reinstatement so your Maryland filing can proceed. Both quoted separately in your written estimate.
How to send your Certificate of Good Standing: After submitting this form, please email your certificate as a PDF attachment to info@mdforfeited.com with your business name in the subject line.
